While communication is necessary to achieve the right outcome, coordination of services is critical for the patient's healthcare plan. 

The following information is required for all referrals 

• Patient's demographics/Facesheet
• Clinical notes, not older than 6 months and/or evaluation
• Diagnosis code- I89.0, I97.2 and C50.919 (most common diagnosis)
• For breast cancer and lymphedema patients, UHC requires a Letter of Medical Necessity for the use of garments
• Order Form with garment recommendations (if applicable)

Pearl Rose Wellness is a multidisciplinary DME that offers comprehensive garment solutions. Our professional, experienced board-certified fitter understands the impact of guiding the patient through the benefits of wearing the right compression garments and be fitted correctly. In addition, our goals are to support the patient and help them understand this process during their journey. 

During the consultation, we'll assess the patient based on their diagnosis, measure and custom fit with the correct garment recommendations, take the necessary time to explain the use of garments, maintenance and care of the garments, and follow-up with patient after the first 6 months for replenishment of garments, if deemed necessary. 

*Providing the right tools for easy donning and doffing is key to compliance and we are committed to provide easy and affordable tools to help the patients during the application of the compression garments and encourage the patient to be participant in their care plan. 

Patient has the following options for compression fitting consultations:

• In-office consultation at our location in Brandon, FL ( 3mi. from Riverview, FL  and Valrico, FL; 17mi. from Tampa, FL and 20mi. from Wesley Chapel, FL) same plaza as "The Greater Brandon Chamber of Commerce" building)
• Telehealth/Virtual fitting consultation 
• Place online and over the phone orders

*A prescription is required. At Pearl Rose Wellness, we provide concierge services and assist with prescription through your referral physician.  Please call our office if the patient needs assistance requesting the prescription. 

We know and understand the importance of educating the physician's office staff on a services line that will enhance the patient experience  of your practice. We want to be an extension of your services and our In-service is free of any cost to your practice. 

During the In-Service, office staff receives educational brochures from Pearl Rose Wellness, products samples for the use of the practice , a prescription note pad from us for patient's referrals, and patient pamphlets samples to give to patients during their physician appointment . (All patient pamphlets can be personalized for your practice)
